Learn About PSA Test Numbers for Seniors

Understanding PSA Test Basics for Older Adults

The PSA test, or prostate-specific antigen test, measures a protein produced by the prostate gland. This small, walnut-shaped gland is part of the male reproductive system and sits below the bladder. The PSA test involves a simple blood draw, usually during a routine doctor visit. A laboratory then measures how much PSA is in the blood sample.

For seniors, understanding PSA numbers matters because PSA levels can indicate different health conditions. The prostate naturally produces PSA, and small amounts enter the bloodstream. However, certain conditions—including prostate cancer, benign prostatic hyperplasia (an enlarged prostate), and prostatitis (prostate inflammation)—can cause PSA levels to rise.

PSA test results are measured in nanograms per milliliter, written as ng/mL. A result of 4.0 ng/mL or lower has traditionally been considered "normal" for most men, though this threshold continues to be discussed among medical professionals. Some men with PSA levels below 4.0 may still have prostate cancer, while others with higher levels may not have cancer at all.

The PSA test became widely used in the 1990s as a screening tool. According to the American Cancer Society, prostate cancer is the second most common cancer in American men, with about 1 in 8 men receiving a prostate cancer diagnosis in their lifetime. However, many prostate cancers grow slowly, and some men may never experience symptoms or complications from the disease.

Practical takeaway: Before getting a PSA test, discuss with your doctor what the test measures and what different results might mean. Understanding the basics helps you have informed conversations about your individual health situation and whether screening is right for you.

What Different PSA Numbers Mean

PSA numbers fall into different ranges, and each range carries different meanings. However, it's important to note that PSA numbers alone cannot diagnose prostate cancer or any other condition. The number is just one piece of health information that doctors consider alongside other factors like age, family history, symptoms, and physical examination findings.

A PSA level of 0 to 4.0 ng/mL is generally considered in the normal range for most men. However, some research suggests that even PSA levels below 2.5 ng/mL may be more typical for men without prostate cancer. Conversely, some men with PSA levels in the 4.0 to 10.0 range may not have prostate cancer at all.

PSA levels between 4.0 and 10.0 ng/mL are sometimes called the "gray zone" because results in this range don't clearly point toward cancer or rule it out. Men with PSA numbers in this range may need additional testing, such as a digital rectal exam or imaging studies, depending on their doctor's recommendation and their individual risk factors.

PSA levels above 10.0 ng/mL are less common in men without prostate cancer, though elevated PSA can also result from benign conditions like urinary tract infections or recent ejaculation. For this reason, doctors often recommend waiting several days after certain activities—like bike riding or recent ejaculation—before getting a PSA test, as these can temporarily raise levels.

PSA velocity, the rate at which PSA levels change over time, also provides information. A rapid increase in PSA from year to year may be more concerning than a stable, slowly increasing level. Doctors track PSA trends across multiple years rather than relying on a single test result.

Practical takeaway: Keep records of your PSA test results over time. Bring these records to appointments so your doctor can see the trend in your numbers and discuss what patterns might mean for your health.

Age Considerations and PSA Screening Decisions

Age plays a significant role in how doctors and patients approach PSA screening. The American Cancer Society notes that prostate cancer risk increases with age, with most cases diagnosed in men over age 65. However, this doesn't mean all older men should be screened, nor does it mean screening is necessarily harmful.

For men in their 50s, medical organizations suggest discussing the benefits and risks of PSA screening with a doctor. For men at higher risk—including African American men and men with a family history of prostate cancer—this conversation may be recommended starting at age 40 or 45. For men over 70, particularly those in good overall health with a life expectancy of 10 years or more, the decision to screen requires careful consideration of individual preferences.

One important factor in these age-related recommendations comes from major research studies. The Prostate, Lung, Colorectal and Ovarian Cancer Screening Trial, which followed men for about 13 years, found that screening with PSA did not reduce prostate cancer death rates compared to usual care. This research influenced recommendations from organizations like the U.S. Preventive Services Task Force, which currently recommends shared decision-making about PSA screening rather than routine screening for all men.

Shared decision-making means having a conversation with your doctor about your personal values, health goals, and concerns. Some men prefer to know their PSA numbers and pursue additional testing if needed, while others would rather not undergo screening. Both approaches can be reasonable, depending on individual circumstances.

Life expectancy matters in these decisions. If a man has a life expectancy of less than 10 years due to other health conditions, the potential benefits of prostate cancer screening decrease. Conversely, a man in excellent health at age 70 with expected longevity into his 80s or 90s might consider screening more relevant to his situation.

Practical takeaway: Schedule a conversation with your doctor about your personal risk factors, health goals, and life expectancy before deciding whether PSA screening makes sense for you. This discussion should happen before any test is performed, not after.

Factors That Affect PSA Numbers

PSA levels fluctuate naturally and can be influenced by various activities and conditions that have nothing to do with prostate disease. Understanding these factors helps explain why a single PSA test result may not be definitive and why doctors recommend retesting if an initial result is unexpectedly elevated.

Recent ejaculation is one of the most common temporary elevators of PSA levels. Studies show that PSA can remain elevated for 24 to 48 hours after ejaculation. For this reason, doctors typically recommend abstaining from ejaculation for at least two days before a PSA test. Vigorous exercise, particularly cycling, can also temporarily raise PSA levels because pressure on the perineal area (the area between the genitals and anus) stimulates PSA release into the bloodstream.

Urinary tract infections and prostatitis cause PSA levels to rise significantly. If a man has symptoms of infection—such as painful urination, frequent urination, or fever—a doctor may recommend delaying PSA testing until after the infection has been treated and fully resolved. PSA levels typically return to baseline within several weeks after infection clears.

Certain medical procedures also elevate PSA temporarily. Digital rectal exams, catheterization, and urinary tract procedures can cause PSA to increase for several days afterward. A prostate biopsy can elevate PSA for four to six weeks. For these reasons, doctors typically recommend waiting at least a week after any urological procedure before drawing blood for a PSA test.

Medications like finasteride (Proscar) and dutasteride (Avodart), which are used to treat enlarged prostate, actually lower PSA levels. If a man is taking these medications, his baseline PSA level will be lower than it would be without treatment. A doctor will factor in medication use when interpreting results.

Practical takeaway: When scheduling a PSA test, inform your doctor about recent infections, procedures, medications, and vigorous exercise. Follow any pre-test instructions your doctor provides to get the most reliable results.

Understanding Your Test Results and Follow-Up Steps

After a PSA test, your doctor will review the results with you and discuss what they mean for your situation. During this conversation, ask about the specific number, how it compares to previous results if available, and what your doctor recommends next. Results don't automatically mean anything is wrong; they're one piece of health information that requires professional interpretation.
